Job Overview
Position Summary: The Youth Support Services Coordinator is responsible for fostering supportive connections with young individuals (ages 5 – 21) and their families facing significant behavioral and/or physical health challenges. The goal is to assist them in achieving enhanced overall health and improved social functioning.
Key Responsibilities:
- Exhibit a comprehensive understanding of childhood psychiatric disorders, chronic health issues, and complex trauma.
- Engage in outreach initiatives utilizing various strategies to connect with individuals experiencing chronic health conditions, mental health issues, and/or substance use disorders, often occurring simultaneously.
- Perform initial and ongoing thorough assessments (CANS-NY, Complex Trauma Assessment, Comprehensive Assessment, etc.) to identify strengths and needs.
- Develop and update care plans that align with member needs and personal aspirations, emphasizing health and wellness maintenance.
- Maintain regular contact with members, providing telephonic and/or in-person outreach, engagement, and comprehensive service planning.
- Advocate for members to ensure they have access to essential resources that promote wellness/self-management and reduce emergency room visits and hospital admissions.
- Oversee and coordinate all aspects of care for members, including medical insurance access and connections to treatment providers and community resources.
- Collaborate with community service providers as part of a multidisciplinary team to ensure effective care planning.
- Provide crisis intervention as necessary and follow up appropriately.
- Keep detailed, timely, and accurate records in an electronic medical record system.
- Coordinate with the Program Coordinator/Supervisor and health home outreach team to ensure accurate caseload management.
- Complete all required monthly documentation to maintain continuity of engaged members' medical insurance and ensure proper billing.
- Work collaboratively within a care management team, participating in team meetings to provide insights and share resources related to member needs.
- Manage daily office and field schedules in coordination with team members and the Program Coordinator/Supervisor.
- Offer resources and serve as a consultant to team members on medical, psychosocial, substance use issues, and social service needs.
- Deliver Youth Care Management services to youth across designated regions as assigned.
- Attend training sessions to enhance skills and meet training requirements.
- Engage in regular individual supervision to address concerns and foster skill development.
- Participate in weekly High Risk Review meetings to monitor member concerns and high-risk behaviors.
- Attend monthly Youth Care Management meetings to discuss agency policy updates, training, and resources.
- Responsible for the upkeep and documentation of agency vehicles when assigned.
- Manage agency-issued equipment responsibly.
- Adhere to program guidelines as specified in the personnel manual.
- Report any issues or concerns to program administration as needed.
- Ensure access to a personal vehicle with insurance coverage.
- Track and submit mileage accurately.
- Other responsibilities as assigned.
Qualifications:
A Bachelor’s degree in Arts or Science with two years of relevant experience (care coordination, youth, and families), or a Registered Nurse license with two years of relevant experience, or a Master’s degree with one year of experience. *Must maintain CANS-NY certification. *Must possess a valid driver's license and car insurance.
