Care Manager MSW

We are seeking a highly skilled Care Manager MSW to join our team at Providence High Desert Medical Group. As a Care Manager, you will play a critical role in providing exceptional patient care and ensuring seamless transitions between healthcare services.
Key Responsibilities- Collaborate with patients, nurses, social workers, physicians, and other healthcare professionals to develop and implement comprehensive care plans.
- Conduct thorough assessments of patients' social and emotional needs, identifying areas for improvement and developing strategies to address them.
- Coordinate with community resources and healthcare providers to ensure patients receive the necessary support and services.
- Communicate effectively with patients and their families, providing education and support to promote healthy behaviors and outcomes.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Master's Degree in Social Work (MSW) or related field.
- 6 months of relevant social work experience in a medical group, health plan, hospital, community, post-acute, or medical office/clinic setting.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
- Certification in Case Management (CCM, ACM) upon hire.
- Care management and managed care experience.
- Behavioral health experience in acute or outpatient settings.
- Utilization management and/or case management experience.
