Job Title: Licensed Practical Nurse

Colorado Visiting Nurse Association is seeking a skilled Licensed Practical Nurse to join our team. As a LPN, you will provide high-quality patient care in a variety of settings, including hospice, palliative care, and home health.

Responsibilities:
  • Collaborate with interdisciplinary teams to develop and implement patient care plans
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions
  • Participate in ongoing education and training to stay current on best practices and industry developments
Requirements:
  • Current Colorado LPN license
  • Minimum one year of experience in hospice, palliative care, or home health
  • Valid Colorado state driver's license and proof of auto insurance
