About the Role

We're seeking a skilled Social Worker MSW to join our interdisciplinary team. As a key member of our hospice care team, you'll provide psychosocial support to patients and their families, utilizing your expertise to identify and address their unique needs and goals.

Responsibilities

Provide comprehensive psychosocial assessments and develop individualized plans of care
Collaborate with the interdisciplinary team to ensure seamless care coordination
Utilize professional judgment and expertise to address complex psychosocial issues
Provide emotional support and guidance to patients and their families during times of crisis

Requirements

Master's Degree in Social Work from a CSWE-accredited program
Licensure as required by the state in which the hospice is located
1+ year of experience as a Social Worker in a healthcare and/or hospice setting
Strong knowledge of hospice care and psychosocial dynamics of illness, loss, and death
