Key Responsibilities:- Work collaboratively with our therapy team to develop and implement individualized treatment plans.
- Provide occupational therapy services to patients in a variety of settings, including skilled nursing facilities.
- Conduct thorough assessments and evaluations to identify patient needs and goals.
- Develop and implement treatment plans that address patient impairments and promote functional independence.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Graduate of an accredited occupational therapy program.
- Current licensure in the state where the SNF is located.
- Previous experience in a rehabilitation setting, preferably in a skilled nursing facility.
