Key Responsibilities:
  • Work collaboratively with our therapy team to develop and implement individualized treatment plans.
  • Provide occupational therapy services to patients in a variety of settings, including skilled nursing facilities.
  • Conduct thorough assessments and evaluations to identify patient needs and goals.
  • Develop and implement treatment plans that address patient impairments and promote functional independence.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
Requirements:
  • Graduate of an accredited occupational therapy program.
  • Current licensure in the state where the SNF is located.
  • Previous experience in a rehabilitation setting, preferably in a skilled nursing facility.
