Job SummaryWe are seeking a highly skilled and compassionate Primary Care Physician to join our team at ChenMed. As a key member of our medical staff, you will be responsible for providing exceptional patient care, leading our care teams, and contributing to the growth and success of our organization.
Key Responsibilities- Provide high-quality, patient-centered care to a panel of up to 450 patients, with a focus on acute and chronic illnesses in older adults.
- Lead your care team, including care promoters, care facilitators, and care coordinators, to ensure seamless patient care and outcomes.
- Develop and implement effective care plans, working closely with patients, families, and other healthcare professionals to achieve optimal health outcomes.
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices that enhance patient care and satisfaction.
- Collaborate with hospitalists, specialists, and other healthcare professionals to ensure comprehensive patient care and coordination.
- Engage with patients, families, and caregivers to promote health education, wellness, and disease prevention.
- Stay up-to-date with the latest evidence-based medicine, clinical guidelines, and best practices in geriatric care.
- Participate in ongoing professional development, including training, education, and certification programs, to maintain expertise and stay current with industry developments.
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
- Current, active State Medical License for the state(s) in which you will be working.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty (Board Eligibility required).
- Basic Life Support (BLS) certification.
- Current DEA number for schedule II-V controlled substances.
