RN Hospice Care Specialist

Job Summary: We are seeking a compassionate and skilled RN Hospice Care Specialist to join our team at Riverside Health System. As a RN Hospice Care Specialist, you will provide high-quality, patient-centered care to individuals with terminal illnesses and their families.
Key Responsibilities:
- Assess, plan, implement, and evaluate the total nursing care of assigned patients on an intermittent visiting basis.
- Develop an initial and ongoing comprehensive assessment of client needs, including the impact of the terminal diagnosis on the patient's physical, functional, psychological, and environmental needs and activities of daily living.
- Assess the patient's risk for pathological grief, cultural and spiritual implications, and verbal/non-verbal communication patterns.
- Accurately complete all required clinical documentation consistently per RHS standards.
- Implement/initiates an individualized plan of care and recommends revisions to the plan when necessary.
- Manage patient's discomfort and provides symptom relief.
- Consults with, councils and educates the patient and family in meeting nursing needs, the disease process, self-care techniques, end of life care and the processes for dealing with issues of ethical concerns.
Requirements:
- Registered Nurse (RN) - State Department of Health Professions (Required)
- CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days (Required)
- Driver's License (Required)
