JOB SUMMARY

The registered nurse is responsible for planning, organizing, and overseeing hospice services, with a strong focus on community health education and experience. This professional role leverages community resources to tailor services that address the unique needs of patients and their families in their homes and local communities.

KEY RESPONSIBILITIES

Patient Care

1. Conducts thorough initial and ongoing assessments of patients and families to identify hospice requirements, including a detailed physical assessment and medical history.

2. Delivers professional nursing care by applying all aspects of the nursing process.

3. Evaluates patient status by:

A. Developing and initiating a comprehensive care plan

B. Regularly reassessing patient and family needs

C. Adjusting the care plan as necessary based on patient status

4. Initiates and modifies the care plan in response to changing patient needs.

5. Utilizes health assessment data to formulate nursing diagnoses.

6. Creates a care plan that sets achievable goals based on nursing diagnoses and includes palliative care actions, involving both the patient and family in the planning.

7. Implements appropriate preventive and rehabilitative nursing procedures, administering medications and treatments as prescribed.

8. Provides counseling to patients and families to address nursing and related needs.

9. Offers health care guidance to patients as per assessment and care plan.

10. Aids patients with daily living activities and promotes self-sufficiency and comfort care.

Communication

1. Maintains accurate clinical documentation regarding patient conditions and care provided, including pain and symptom management updates.

2. Communicates effectively with physicians regarding patient needs and changes in condition, obtaining necessary orders.

3. Collaborates with community health professionals to coordinate care plans.

4. As a mandatory reporter, adheres to state and federal regulations regarding reporting requirements.

5. Reports any suspected cases of abandonment, abuse, or neglect to the appropriate authorities immediately.

6. Educates patients and families on self-care techniques and provides necessary instructions regarding medications and diet.

7. Ensures a safe environment for patients.

8. Supports continuity of care by assisting patients, families, and team members.

9. Works collaboratively with families and the interdisciplinary team to address emotional needs.

10. Participates in interdisciplinary meetings.

Additional Responsibilities

1. Participates in on-call duties as outlined in the policy.

2. Ensures availability of necessary equipment and services.

3. Supervises support staff and delegates tasks as needed.

4. Commits to personal and professional development through continuing education and training.

5. Completes assigned case responsibilities as required.

6. Engages in quality assessment and performance improvement initiatives.

QUALIFICATIONS

1. Graduate of an accredited nursing program.

2. Active Registered Nurse license in good standing in Virginia.

3. Minimum of one year of nursing experience within the last three years, or a baccalaureate degree in nursing with at least two years of experience, preferably in public health, home care, or hospice nursing.

4. Management experience is not mandatory but may involve supervising hospice aides.

5. Employment is contingent upon satisfactory references and background checks.

6. Strong observational, verbal, and written communication skills, along with problem-solving abilities.

7. Physical ability to perform nursing tasks, including lifting and transferring patients.

8. Must possess a valid driver's license and have access to a reliable, insured vehicle.
