Job Summary

We are seeking a skilled Case Manager to join our team at Mental Health Systems, Inc. in East SUD. As a Case Manager, you will play a vital role in providing comprehensive support to clients, ensuring they receive the necessary resources and services to achieve their goals.

Key Responsibilities
  • Provide case management services, including crisis intervention, discharge planning, and group and individual work.
  • Assist clients in meeting their basic needs and managing symptoms.
  • Complete accurate and timely documentation, meeting minimum billable requirements for direct client services.
  • Facilitate placement, including pre-placement visits and transportation as needed.
  • Shift coverage, including part of the on-call team.
  • Provide field-based services as required by program and client need.
  • Maintain confidentiality of client care.
  • Participate in scheduled program meetings.
Requirements
  • Master's degree with two years of experience in social services, or bachelor's degree with four years of experience in social services.
  • Certification from a recognized board is preferred and registration required for SUD CM.
  • Valid CA driver's license and current auto insurance per contract requirements.
  • Experience providing mental health and/or substance use disorder services preferred.
  • Cultural competency understanding required.
  • Bilingual preferred.
  • Knowledge of Microsoft Office and EHR software preferred.
