Pernet Family Health Service, Inc. is seeking a highly skilled Early Intervention Physical Therapist to join our team. As a member of our multidisciplinary team, you will provide developmental screening, assessment, direct service, and guidance to families of children birth to three years of age who are at risk for, or experiencing developmental delays.
Responsibilities- Provide home-based services to families across the catchment area.
- Demonstrate knowledge and understanding of all areas of infant-toddler development.
- Function as a multi-disciplinary team member in the screening and developmental assessment of children birth to three years of age who are at risk for or experiencing developmental delays.
- Participate in screenings, assessments, child groups, parent groups, center visits, and home visits as a member of a multidisciplinary team.
- Utilize a parent coaching model in working with families.
- Provide service coordination to assigned caseload including coordinating the development, monitoring, and review of the Individual Family Service Plan.
- With parental permission, collaborate with other community agencies to ensure continuity of services for families including childcare providers, medical providers, social service agencies, and specialty providers serving children with autism or hearing/vision loss to support the child and the family; empower families to advocate for their rights and needs.
- Current licensure as a Physical Therapist by the Massachusetts Board of Registration of Allied Health Professions.
- Two (2) years of pediatric experience is preferred.
- Effective verbal and written communication skills.
- Working knowledge of Google Drive, Word, Excel, and Office 365 including Outlook, OneDrive, Zoom, and Teams.
- Ability to use computer-based technology including a computer/laptop and cell phone.
- Valid driver's license and reliable transportation is required.
