Job Summary:The Registered Nurse Case Manager will be responsible for managing a defined caseload of patients, providing direct patient care and comprehensive nursing services to individuals in the community.
Key Responsibilities:- Manage a caseload of patients, providing direct care and support
- Conduct thorough assessments and develop individualized care plans
- Collaborate with healthcare teams to ensure seamless care transitions
- Provide education and support to patients and their families
- Monitor patient progress and adjust care plans as needed
- Valid nursing license in the state of New Hampshire
- 2+ years of experience in skilled care, preferably with wound care experience
- 1+ year of experience in nursing supervision, preferred
- Current CPR certification
- Reliable transportation
