Job Title: Registered Nurse Case Manager

Job Summary: We are seeking a skilled Registered Nurse to join our team as a Case Manager. The ideal candidate will have experience in skilled care and be able to provide direct patient care and comprehensive nursing services to individuals in the community.

Responsibilities:

  • Manage a defined caseload of patients
  • Provide direct patient care and comprehensive nursing services
  • Collaborate with healthcare teams to develop and implement patient care plans
  • Conduct regular assessments and evaluations to ensure patient needs are met
  • Communicate effectively with patients, families, and healthcare teams

Requirements:

  • Valid nursing license in the state of NH
  • 2+ years of experience in skilled care
  • 1+ year of experience in nursing supervision
  • Recent wound care experience
  • Ability to lift 50 LBS with no restrictions
  • Current CPR certification
  • Reliable transportation
