We are seeking a compassionate and skilled Home Health Registered Nurse to join our team at Interim HealthCare. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes, working closely with a multidisciplinary team to ensure seamless care transitions.
Key Responsibilities:- Provide comprehensive home-based care to patients with various medical conditions, including illnesses, injuries, and chronic diseases.
- Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans.
- Conduct thorough assessments, document progress, and report changes to the patient's physician.
- Educate patients and their families on medication management, therapy, diet, and exercise.
- Ensure timely and effective discharge planning to prevent hospital readmissions.
- Active RN license in Colorado.
- Minimum of one year of nursing experience, preferably in home healthcare.
- Wound care experience a plus.
- CPR certification required.
- Pass a background check and drug test.
