We are seeking a compassionate and skilled Hospice LPN to provide high-quality patient care in a dynamic and supportive environment.

Key Responsibilities:
  • Provide direct patient care as defined by the State Nurse Practice Act
  • Implement current nursing practice following a comprehensive assessment and the plan of care
  • Document patient services accurately and timely to reflect the plan of care
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
  • Participate in the coordination of hospice services, reporting identified needs to the interdisciplinary group
  • Provide appropriate pain/symptom management, evaluating patient response to treatments/medications
Requirements:
  • Graduate of an accredited practical nurse or vocational nursing program
  • Three years of nursing experience, with community health, hospice, or medical/surgical experience preferred
  • Currently licensed as an LPN/LVN in the practicing state
  • Must possess current CPR Certification
  • Demonstrates good verbal and written communication, and organization skills
  • Valid driver's license with an automobile that is insured in accordance with state requirements
  • Understands the philosophy of hospice concept and the needs of the terminally ill
