Job Summary

We are seeking an exceptional RN Case Manager to join our team. As a key member of our interdisciplinary team, you will provide skilled nursing care, case management services, and conduct initial assessments of new patients. You will also travel to patients' homes, nursing facilities, and other locations to provide care.

Responsibilities
  • Provide skilled nursing care and case management services to patients and their families.
  • Conduct initial assessments of new patients to determine their needs and develop a comprehensive care plan.
  • Travel to patients' homes, nursing facilities, and other locations to provide care.
  • Participate in on-call schedules and provide 24/7 coverage.
  • Collaborate with the interdisciplinary team to develop and implement patient care plans.
  • Document patient care and progress in a timely and accurate manner.
Requirements
  • Valid Registered Nurse (RN) license.
  • Driver's license, insurance, and vehicle.
  • 1-2 years of nursing experience (preferred).
  • Utmost compassion and integrity.
