About the Role

We are seeking a skilled and compassionate Home Health Registered Nurse to join our team at CenterWell Primary Care. As a key member of our care team, you will play a vital role in delivering high-quality, patient-centered care to individuals in their homes.

Responsibilities
  • Provide comprehensive nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Conduct regular home visits to assess patient needs, provide education, and coordinate care with other healthcare professionals.
  • Collaborate with physicians, therapists, and other healthcare professionals to ensure seamless care coordination.
  • Document patient progress, care plans, and other relevant information in a timely and accurate manner.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Diploma, Associate, or Bachelor's degree in Nursing.
  • Minimum of one year of nursing experience, preferably in home health or a related field.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
  • Strong organizational and communication skills.
  • A valid driver's license, auto insurance, and reliable transportation.
