Job Summary

We are seeking a skilled Physical Therapist to join our team of healthcare professionals. As a Physical Therapist with HarmonyCares, you will have the opportunity to make a meaningful difference in the lives of our patients, providing personalized care and support to help them achieve their goals.

Responsibilities
  • Provide comprehensive physical therapy services to patients in their homes, including assessment, evaluation, and treatment planning.
  • Communicate effectively with patients, families, and other healthcare professionals to ensure seamless care coordination.
  • Supervise and instruct non-professional home health aide staff in the provision of personal care to patients.
  • Update care plans regularly to reflect changes in patient needs and progress.
  • Submit completed visit notes and participate in ongoing quality improvement initiatives.
Requirements
  • Current, unencumbered State Physical Therapy License in the state of practice.
  • One year of experience as a home care Physical Therapist, with proficiency in clinical skills and Microsoft Office.
  • Ability to make sound professional clinical judgment and assess patient needs.
  • Reliable mode of transportation and a valid driver's license.
