Job Summary

We are seeking a skilled and compassionate RN/LVN to join our team as a Pediatric Home Health Nurse. As a member of our team, you will have the opportunity to work with a diverse range of clients, providing high-quality care and support in the comfort of their own homes.

Responsibilities

  • Provide skilled nursing care to pediatric clients in their homes, including wound care, medication management, and vital sign monitoring
  • Develop and implement individualized care plans to meet the unique needs of each client
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication
  • Provide education and support to clients and their families on disease management, medication adherence, and healthy lifestyle habits
  • Conduct thorough assessments of clients' physical and emotional needs, identifying areas for improvement and developing strategies for enhancement

Requirements

  • Current RN or LVN licensure in the state of practice
  • Current CPR certification
  • Experience working with pediatric clients, preferably in a home health setting
  • Strong communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare teams
  • Ability to work independently and as part of a team, with a strong commitment to delivering exceptional patient care
