Job Summary

We are seeking a Hospice RN Case Manager to provide excellent care to our patients along with our acute care partners. As a Hospice RN, you will work with patients, their families, and other healthcare professionals during the final stages of patients' lives.

Responsibilities
  • Provide compassionate care to patients and their families in private homes, residential care facilities, nursing centers, and other hospice care environments.
  • Supervise licensed vocational/practical nurses and nursing aides as needed.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care.
  • Monitor patients' conditions and alert doctors and others as necessary.
Qualifications
  • Graduate of an approved school of nursing and licensed as a Registered Nurse in the state of employment.
  • Valid state driver's license, automobile liability insurance, and dependable transportation.
  • Minimum of one year of clinical experience, with previous home health or hospice experience preferred.
