At Enhabit Home Health & Hospice, we are committed to delivering exceptional care to our patients. As a Registered Nurse (RN) Case Manager, you will play a vital role in ensuring the highest quality of care is provided to our patients in the comfort of their own homes.
Key Responsibilities- Perform home visits and coordinate client care to ensure the best possible outcomes for our patients.
- Act as the primary point of contact for all disciplines involved in patient care, overseeing the frequency of visits and ensuring seamless communication with physicians and office staff.
- Consult with physicians and office staff as needed to provide detailed updates on patient care and ensure timely reporting of lab values and medication profiles.
- Attend weekly case conferences and monthly case manager meetings to stay informed and contribute to the development of effective care plans.
- Collaborate with the care team to ensure accurate OASIS collection, medication profile management, and timely reporting of lab values.
- Current licensure as a Registered Nurse (RN) in the state of employment.
- A minimum of one year of clinical experience, with a preference for experience in home health care.
- Demonstrated knowledge and skill in current nursing practice, with a focus on patient-centered care.
- A valid state driver's license and automobile liability insurance.
