**Job Summary:**

The Patient Care Facilitator plays a critical role in overseeing the clinical aspects of Wound Care and Vascular Access at St. Bernards Medical Center. This position requires a registered nurse with current state licensure and a minimum of five years of experience, preferably in an ambulatory setting. The ideal candidate will have prior management experience and a strong understanding of the State Nurse Practice Act.

**Key Responsibilities:**

* Collaborate with the Vice President to coordinate services and provide quality care to patients with wounds and IV access
* Work within the framework of the State Nurse Practice Act and follow hospital policies and procedures
* Coordinate operational clinical systems with the Vice President

**Requirements:**

* Registered Nurse with current state licensure
* Minimum five years of experience, preferably in an ambulatory setting
* Prior management experience
* Strong understanding of the State Nurse Practice Act

**Work Environment:**

This is a safety-sensitive position. The Patient Care Facilitator will work in the Outpatient Wound Healing Center and be required to stand, walk, use hands to finger, handle or feel, reach with hands and arms, and talk or hear. The employee must frequently lift and/or move up to 50 pounds and occasionally lift and/or move patients. Specific vision abilities required by this job include close vision, distance vision, and color vision.
