Chaplain - Hospice and Palliative Care Job Overview

Affinity Care of Ohio is seeking a qualified Chaplain - Hospice and Palliative Care to join our team. As a chaplain, you will provide spiritual support and guidance to our patients and their families, working closely with our interdisciplinary team to ensure comprehensive care.

Job Summary

We are looking for an experienced and ordained chaplain who can provide spiritual support and guidance to our patients and their loved ones. The successful candidate will be responsible for assessing the spiritual needs of our patients, developing individualized plans of care, and providing direct spiritual care. Additionally, they will serve as a liaison between our team and community chaplains, spiritual counselors, and other healthcare professionals.

Key Responsibilities
  • Provide spiritual assessment and care to patients and their families
  • Develop and implement individualized plans of care that meet the spiritual needs of our patients
  • Serve as a liaison between our team and community chaplains, spiritual counselors, and other healthcare professionals
  • Maintain accurate records of spiritual care services provided
  • Attend patient care conferences and interdisciplinary group meetings as a member of the interdisciplinary team
Qualifications and Requirements
  • Bachelor's degree in religious studies or a related field and master's degree in theology, divinity, or pastoral studies required
  • Certification as a chaplain preferred
  • Experience in hospice care and interfaith settings desirable
  • Ability to communicate effectively with patients, families, and healthcare teams
  • Strong organizational and time management skills
Estimated Salary: $75,000 - $90,000 per year
