Pharmacist Consultant

We are seeking a highly skilled Pharmacist Consultant to join our team at Guardian Pharmacy. As a key member of our long-term care pharmacy services team, you will play a critical role in delivering exceptional care to our patients.

• Evaluate and optimize medication regimens for patients in long-term care facilities to ensure safe and effective treatment.
• Monitor compliance with federal and state regulations, identifying areas for improvement and implementing corrective actions.
• Serve as a trusted advisor to facility staff, providing education and guidance on medication management and regulatory requirements.
• Collaborate with interdisciplinary teams to develop and implement care plans that meet the unique needs of each patient.
• Conduct regular audits and reviews to ensure accurate and compliant medication management practices.

• Pharmacist license in good standing.
• 2+ years of experience in long-term care pharmacy or a related field.
• Advanced computer skills and experience with pharmacy operations systems.
• Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
