Overview

Novant Health is seeking a skilled nursing professional to join our home health team in Wilmington, NC. As a registered nurse, you will be responsible for coordinating home care services, including establishing and maintaining patient plans of care.

Responsibilities
  • Conduct admissions, recerts, ROC, and discharges for assigned caseloads
  • Perform routine nursing visits as needed
  • Assess patient needs and coordinate care between patients, caregivers, physicians, and other disciplines
  • Complete all home health documentation
Requirements
  • Graduate of an accredited school of nursing
  • Associate's or Bachelor's degree in nursing
  • Current valid North Carolina nursing license
  • Basic Life Support (BLS) through American Red Cross or American Heart Association
  • Driver's License Class C
  • Minimum 1 year of home health or related experience
