About the Role

We are seeking a compassionate and skilled Nurse Practitioner to join our team in providing high-quality medical care to seniors and individuals with complex needs in the comfort of their own homes.

Responsibilities
  • Provide evidence-based medical care to patients, including examining and treating chronic and urgent episodic illnesses, minor injuries, and other health issues.
  • Order and interpret laboratory and diagnostic testing as needed, and collaborate with caregivers and other healthcare professionals to ensure comprehensive care.
  • Develop and implement individualized care plans, and provide referrals to other healthcare practitioners when necessary.
  • Ensure accurate and up-to-date patient medical records, and maintain a strong knowledge base of HIPAA and OSHA regulations.
Requirements
  • Active state Nurse Practitioners license
  • Current Nurse Practitioner National Board Certification
  • Active state DEA licensure
  • Master's degree in Nursing (MSN)
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
