Job Overview

Position Summary:

The Nurse plays a vital role within the patient-centered medical home team. This position is accountable for overseeing individualized patient care, focusing on enhancing and restoring the health of patients through the nursing process. The Nurse collaborates with healthcare providers and multidisciplinary team members, providing both physical and emotional support to patients.

Qualifications:

  • Diploma in nursing with a minimum of two years of relevant experience.
  • Current licensure from the South Carolina State Board of Nursing.
  • Certification in CPR (Cardiopulmonary Resuscitation) or BLS (Basic Life Support) is mandatory.
  • Exceptional customer service skills and strong organizational abilities are essential.
  • At least two years of experience in a primary or specialty care medical environment.
  • Capability to perform effectively in a high-pressure setting.

  • Ability to engage with patients, families, clinical and non-clinical staff, and providers from diverse backgrounds while maintaining a non-judgmental demeanor.
  • Excellent verbal and written communication skills are required.
  • Proficient in collaborating with others to enhance quality and address patient needs.
  • Strong initiative and the ability to manage multiple tasks concurrently.
  • Familiarity with policies, procedures, regulations, and protocols is necessary.
  • Knowledge of database systems, such as EMR systems.
  • Bilingual proficiency in English and Spanish is preferred.
  • Current knowledge of medical practices, including pharmacology, is required.
  • At least two years of experience in a physician's office setting.
  • Outstanding interpersonal skills and attention to detail.
  • Ability to multitask effectively.
  • Compassionate approach towards patient care.
  • Proficient in administering various treatments and medications as prescribed.
  • Adaptability to new computer software.

Supervisory Role:

  • This position does not have direct supervisory responsibilities.

Core Responsibilities:

  • Evaluate patient needs based on data collected through history, observation, physical examination, telephone triage, and diagnostic analysis.
  • Deliver direct patient care, including triage, assessment, procedures, treatment, patient education, and follow-up instructions.
  • Assist patients and providers in securing medicinal prior authorizations as necessary.
  • Ensure patients comprehend the purpose of their visit.
  • Provide preventative care information tailored to the patient's age, gender, and health status.
  • Maintain knowledge of practical nursing principles to deliver effective patient care and treatment.
  • Understand examination, diagnostic, and treatment room procedures.
  • Interpret, adapt, and apply guidelines and procedures effectively.
  • Engage in basic critical care and chronic illness management.
  • Participate in training opportunities as required.
  • Contribute to enhancing office efficiency and patient encounter volumes.
  • Administer vaccines and immunizations to pediatric and adult patients.
  • Manage daily clinical workflows, including referral management, telephone encounters, lab results, medication refills, and compliance with sample medication logs.
  • Willingness to learn about AIDS Service Organizations and Community Health Centers.
  • Assist in onboarding new nurses to the organization, ensuring they understand standard operating procedures.
  • Perform all other duties as assigned.

Physical Demands:

Hearing: Sufficient to perform job duties in person and over the phone. Speaking: Must communicate clearly with patients in person and via telephone. Vision: Adequate visual acuity to perform job duties, including reading printed materials and computer screens. Other: Requires occasional lifting and carrying of items weighing up to 10 pounds unassisted during daily activities and up to 30 pounds for other required tasks. Frequent bending, reaching, and repetitive hand movements are necessary, along with standing, walking, squatting, and sitting, with regular lifting, pushing, and pulling throughout the work shift.
