Key Responsibilities
  • Collaborate with healthcare professionals to develop and implement comprehensive care plans
  • Provide psychosocial support and education to patients and caregivers
  • Conduct home visits and assessments to identify patient needs
  • Develop and maintain a database of community resources for patients
  • Communicate effectively with patients, families, and healthcare teams
Requirements
  • Master's degree in social work, counseling, or nursing
  • Knowledge of APS mandatory reporting laws
  • 2+ years of experience in social services or discharge planning
  • Palliative care experience preferred
  • Strong communication and advocacy skills
