KEY RESPONSIBILITIES:

  • Facilitate the planning, development, organization, implementation, evaluation, and management of the Social Services Department in alignment with current Federal, State, and local regulations.
  • Establish and oversee initial interactions with discharge planners, families, or responsible parties to coordinate facility tours and follow-up actions.
  • Contribute to the creation and execution of policies and procedures aimed at identifying the medically-related social and emotional needs of residents.
  • Foster and maintain effective working relationships with intra-departmental staff, other facility departments, and external community health, welfare, and social service organizations to ensure comprehensive social service programs are sustained to meet resident needs.
  • Stay informed about current federal and state regulations as well as industry standards.
QUALIFICATIONS:
  • Possession of a valid Social Work license, in good standing with the state.
  • Bachelor's Degree in Social Work is highly preferred.
  • A minimum of 3 years of experience in a long-term care setting is required.
  • Ability to effectively organize and prioritize multiple tasks.
  • The ideal candidate will demonstrate strong communication, leadership, and motivational skills.
