Job Summary:
We are seeking a skilled Hospice Social Worker to join our team at ViaQuest Hospice. As a Hospice Social Worker, you will play a vital role in providing emotional and psychological support to patients and their families.

About the Role:
In this position, you will be responsible for performing comprehensive patient assessments, providing patient visits as scheduled, and offering short-term crisis intervention and individual or family counseling as needed. You will also utilize our electronic medical record system, Brightree, to complete documentation.

Requirements:
To be successful in this role, you will need to have a Master's degree in Social Work (MSW) or a Bachelor's degree in Social Work (BSW), with a valid license in Social Work (LSW). Experience in Medical Social Work is preferred, as well as experience within the Hospice field. Additionally, you will need to have a valid driver's license, car insurance, and reliable transportation.

Estimated Salary:
Based on industry standards and your level of experience, the estimated salary for this position is $65,000 - $80,000 per year, depending on location and qualifications.
