As a Community Health Worker, you will provide peer-to-peer support to families and children with special healthcare needs, connecting them with essential resources and services.
Key Responsibilities- Conduct family assessments and develop individualized family care plans to address the unique needs of each family.
- Provide care coordination and support to families, empowering them to navigate complex healthcare systems.
- Collaborate with other service providers to ensure seamless connections and referrals.
- Act as a liaison between families and healthcare providers, advocating for the needs of children with special healthcare needs.
- Participate in statewide system analysis, planning, and coordination to improve healthcare outcomes for families with special needs.
- High school diploma or equivalent required; associate's or bachelor's degree in human services, education, or a related field preferred.
- Experience supporting families and individuals with special healthcare needs required.
- Demonstrated life experience as a consumer, parent, or family member of a consumer with special healthcare needs.
- Fluency in English and Spanish required.
The primary work location is a climate-controlled indoor office environment. However, you may also be required to conduct visits in private homes, community locations, and other settings. A valid driver's license and reliable transportation are necessary for home and community visits.
