Job Summary

We are seeking a compassionate and dedicated Case Manager to join our team at Pediatric Home Healthcare. As a key member of our staff, you will be responsible for ensuring the highest quality of patient home care through direct case management.

Key Responsibilities
  • Perform and document pediatric patient assessments to ensure thorough and accurate documentation.
  • Ensure that documentation submitted to insurance is thoroughly completed and compliant with regulatory standards.
  • Conduct Skills Reviews for field nurse applicants to ensure they meet our high standards.
  • Monitor and maintain continuity, quality, and safety of all services delivered.
  • Promote the philosophy and administrative philosophies of our organization to ensure the best possible patient care.
Requirements
  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Minimum 2 years' experience in pediatric, NICU, or home healthcare nursing.
  • Minimum 1 year's experience in pediatric home healthcare is strongly preferred.
  • Current Healthcare Provider BLS from American Red Cross or American Heart Association (or subsidiary).
  • Strong English language skills; bilingual in Spanish preferred.
