Salary:

$150-$250 per completed in-person visit; $75-$100 per completed telehealth session

Job Description:

Field Team Clinician Role: This position involves delivering in-home clinical services as part of an integrated multidisciplinary care team. You will build meaningful relationships with patients and their caregivers, supporting them in aligning patient care with personal values and preferences.

Key Responsibilities:

  • Comprehensive care delivery, including urgent care and chronic condition management
  • Collaboration with the multidisciplinary team for transitions of care support, social determinants interventions, behavioral health evaluation, and medication management
  • In-person and virtual clinical care delivery across various settings (private homes, congregate living facilities, ambulatory clinics)
  • Integration of clinical practices, data dashboards, and operational processes into daily workflows
  • Patient engagement and experience initiatives
  • Maintenance of comprehensive medical records and timely documentation

Qualifications and Skills:

  • Nurse Practitioner or Physician Assistant with graduation from an accredited school and national board certification
  • Active licensure and DEA registration
  • Geriatric training preferred; basic life support certification required
  • Experience caring for aging adults and those with complex needs, preferably in home settings
  • Average to advanced computer skills and adaptability
