About the Role

AIDS Healthcare Foundation is seeking a highly skilled Medical Care Manager to join our team. As a Medical Care Manager, you will play a critical role in ensuring that our clients receive the highest level of care and support.

Key Responsibilities:

  • Conduct Needs Assessments and develop Action/Care Plans to ensure clients receive the necessary resources and support.
  • Provide education and assistance to clients to understand their diagnoses and medications.
  • Collaborate with healthcare providers and other stakeholders to ensure seamless care coordination.
  • Facilitate referrals and support for clients who lack access to basic needs such as housing, food, and clothing.
  • Meet with clients to discuss their progress and make adjustments to their care plans as needed.

Requirements:

  • Experience in case management and patient advocacy.
  • Strong knowledge of the Ryan White Care Act Program and its requirements.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize multiple tasks.
  • Passion for working with vulnerable populations and commitment to AHF's core values.
