Job Summary

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team as a Private Duty LPN. As a key member of our home care team, you will provide personalized care to patients in their homes, making a meaningful difference in their lives.

Responsibilities
  • Deliver high-quality, one-on-one care to patients in their homes, promoting independence and comfort.
  • Develop and implement individualized care plans, utilizing sound judgment and critical thinking skills.
  • Maintain accurate and detailed documentation, adhering to our agency's policies and procedures.
  • Perform all skills, treatments, and procedures within the scope of practice defined by the Nurse Practice Act in the state of clinical practice.

Requirements
  • Unencumbered LPN license in the state where care is provided.
  • Minimum one year of nursing experience.
  • Excellent problem-solving skills, organizational, and multi-tasking abilities.
  • In-depth knowledge of health and safety guidelines and procedures.
  • Valid driver's license, auto insurance, BLS CPR card, and up-to-date TB skin test or chest X-ray.
  • Ability to stand, walk, squat, bend, kneel, reach, twist, push, and pull.
  • Ability to lift or transfer up to 50 pounds.
