Key Responsibilities
  • Provide clinical leadership and guidance to rehabilitation staff, including training, mentoring, and performance management.
  • Develop and implement care plans that meet the unique needs of each patient, in collaboration with the interdisciplinary team.
  • Conduct initial assessments and establish treatment plans to achieve optimal patient outcomes.
  • Collaborate with physicians and other healthcare professionals to ensure seamless care coordination and communication.
  • Manage patient caseloads, including evaluating patients, ensuring compliance with established care plans, and performing joint visits as needed.
  • Stay up-to-date with relevant laws, regulations, and industry standards, ensuring compliance and adherence to best practices.
  • Develop and maintain accurate and comprehensive patient records, in accordance with agency, state, and federal guidelines.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements
  • Master's degree in Physical Therapy or Occupational Therapy, or related field.
  • Current licensure as a Physical Therapist or Occupational Therapist in the state of Wisconsin.
  • Basic Life Support (BLS) certification, issued by the American Heart Association.
  • Valid driver's license and reliable transportation.
  • Minimum 5 years of experience in physical therapy or occupational therapy, including experience in home health care.
Preferred Qualifications
  • Experience working in a home health care setting, with a focus on rehabilitation services.
  • Knowledge of relevant laws, regulations, and industry standards, including those related to home health care and rehabilitation services.
  • Strong leadership and communication skills, with the ability to work effectively with interdisciplinary teams.
  • Ability to work independently and make sound judgments, with a focus on patient-centered care.
Working Conditions

This role requires the ability to work safely with people of various sizes, weights, and medical conditions, and to perform physical tasks that may involve lifting, bending, and standing for extended periods.

The successful candidate will be required to provide proof of a valid driver's license and reliable transportation, as well as maintain accurate and comprehensive patient records, in accordance with agency, state, and federal guidelines.
