Job Summary

The Registered Nurse (RN)-Home Health is a vital member of our healthcare team, responsible for planning, coordinating, and documenting care for an assigned caseload of patients. This role requires a strong commitment to delivering high-quality patient care and collaborating with interdisciplinary teams to achieve optimal outcomes.

Key Responsibilities
  • Develop and implement individualized home health plans that meet each patient's unique needs and are in compliance with relevant regulations and standards.
  • Collaborate with physicians, case managers, and other healthcare professionals to ensure seamless care coordination and effective communication.
  • Conduct thorough assessments of patients' physical, emotional, and social needs, and develop plans to address these needs.
  • Provide education and support to patients and their families to promote self-care and independence.
  • Participate in Quality Assessment and Performance Improvement (QAPI) activities to drive continuous quality improvement.
Requirements
  • Graduate from an accredited and approved school of nursing accepted for licensure in the state of practice is required.
  • Bachelor's Degree in nursing is preferred.
  • Valid state driver's license, current automobile insurance, and reliable automobile are required.
  • Prior experience in home health or hospice is preferred.
  • 1 year acute nursing experience is preferred.
  • Licensed as a Registered Nurse (RN) in the state of practice is required.
  • Basic Life Support (BLS) is required.
