The Mental Health Association of Monmouth County is seeking a highly skilled Clinical Program Coordinator to join our team. As a key member of our Community Support Services (CSS) program, you will be responsible for providing comprehensive rehabilitation services to individuals with mental health needs.
Key Responsibilities:
- Assessment and Planning: Conduct comprehensive rehabilitation needs assessments, develop individualized rehabilitation plans, and monitor progress to ensure optimal outcomes.
- Case Management: Maintain a caseload and provide case management services as needed, ensuring seamless coordination of care and support.
- Therapeutic Services: Provide therapeutic rehabilitative skill development, crisis intervention, and other services as outlined in N.J.A.C. 10:37B-4.2.
- Service Coordination: Coordinate and manage services to meet consumers' needs, including referrals to community resources and supports.
- Documentation and Reporting: Maintain accurate and timely documentation in consumer files, adhering to policies and applicable regulations.
- Team Collaboration: Participate in supervision meetings and agency staff meetings to ensure effective communication and collaboration with colleagues.
Requirements:
- Education: Master's degree in a mental health field required.
- Licensure: Possession of a valid license, such as Licensed Social Worker (LSW), Licensed Associate Counselor (LAC), Licensed Clinical Social Worker (LCSW), or Licensed Professional Counselor (LPC).
- Experience: Two years' related work experience.
