We are looking for a CNA who is licensed or in the immediate process of obtaining licensure in the state of NH. The ideal candidate will have a minimum of 1 year of recent experience in SNF / LTC and a strong understanding of the importance of incorporating the organization's vision, values, and mission into their job performance.

Responsibilities:
  • Provide high-quality patient care and support to residents in SNF / LTC settings
  • Assist with daily living activities, such as bathing, dressing, and grooming
  • Monitor and report any changes in patient condition to the healthcare team
  • Collaborate with other healthcare professionals to ensure seamless care delivery
Requirements:
  • Current Certified Nursing Assistant (CNA) license within the state of practice
  • 1 year of recent experience as a CNA in SNF / LTC
  • Strong communication and interpersonal skills
