Job Summary

Vaya Health is seeking a highly skilled Innovations Care Manager to join our team. As a key member of our care management team, you will be responsible for providing proactive intervention and coordination of care to eligible Vaya Health members and recipients.

Key Responsibilities
  • Conduct comprehensive bio-psycho-social assessments addressing social determinants of health, mental health history and needs, physical health history and needs, activities of daily living, access to resources, and other areas to ensure a whole person approach to care.
  • Develop and implement person-centered care plans to help members achieve their health and wellness goals.
  • Work collaboratively with members, care teams, and community stakeholders to ensure seamless care coordination and transitions.
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
  • Participate in interdisciplinary care team meetings and contribute to the development of comprehensive care plans.
Requirements
  • Bachelor's degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human services field.
  • Two years of prior Long-term Services and Supports and/or Home Community-Based Services coordination, care delivery monitoring, and care management experience.
  • Ability to work in a fast-paced environment and adapt to changing priorities and deadlines.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and stakeholders.
  • Proficiency in Microsoft Office products and Vaya systems, including the care management platform, data analysis, and secondary research.
Preferred Qualifications
  • Master's degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human services field.
  • Experience working with vulnerable populations, including individuals with mental health and substance use disorders, intellectual and developmental disabilities, and traumatic brain injuries.
  • Knowledge of the NC Medicaid program, NC Medicaid Transformation, Tailored Plans, state-funded services, and accreditation requirements.
