Job Title: Case Manager

Premise Health is seeking a highly skilled and compassionate Case Manager to join our team. As a Case Manager, you will play a critical role in providing clinical, casework, and group work services to our clients.

Job Summary:

The Case Manager will assess the coordination and access to needed medical, psychiatric, social, educational, and other services essential for meeting the health needs of the individual. You will coordinate assessment, treatment planning, and return to work processes with the employee, providers, and employer.

Key Responsibilities:
  • Assess and evaluate cases and prepares and implements treatment and case management plans using benchmarking to guide practices and timelines
  • Coordinates and provides care that is safe, timely, effective, efficient, equitable, and patient-centered
  • Counsels clients, assisting them to identify and resolve problems and make effective use of resources
  • Maintains collaborative working relationships with other community providers to develop specialized services as required
  • Acts as liaison between client employees, providers, and agency or institution
  • Assists clients in finding ways of adjusting to changes in physical capabilities that will allow for a life/work style as comfortable as the situation will permit
  • Promotes quality and cost-effective interventions and outcomes
  • Provides consultation to staff on specific cases
  • Prepares and maintains written case records, reports, and forms, performs case follow-up and closing, and performs other administrative tasks as required
Requirements:
  • Certificate from an accredited Nursing School or Associate Degree (A.S.) required; Bachelor's Degree (BSN) from four-year college or university preferred
  • Currently licensed Registered Nurse in state of practice required
  • Current hands on training in AHA or ARC Basic Life Support for health care providers is required. Advanced Cardiac Life Support may also be required based on contract scope of services
  • Case Management (CCM) Certification, AANC – CM, American Academy of Case Management (AACM), COHN-S/CM, Certification in Occupational Health Nursing, Certification in Emergency Nursing, or Certificate in Case Management preferred
Preferred Qualifications:
  • At least three years' experience as a Registered Nurse in an emergency room, occupational health clinic or other ambulatory care clinic
  • Knowledge of Internet software, Spreadsheet software and Word Processing software
  • Knowledge and experience with Electronic Medical Records preferred
  • Knowledge of workplace health and safety concepts and OSHA regulations preferred
