About the Role

We are seeking a skilled Nurse Practitioner to join our team at ArchCare, where you will have the opportunity to make a meaningful difference in the lives of our patients.

As a Nurse Practitioner, you will be responsible for providing high-quality patient care, conducting assessments, developing care plans, and implementing treatment plans in a variety of settings, including clinics, group homes, nursing homes, and patient homes.

Key Responsibilities:

  • Conduct comprehensive assessments of patients, including obtaining health records, performing physical and mental evaluations, and evaluating diagnostic tests.
  • Develop and implement individualized care plans, taking into account the unique needs and goals of each patient.
  • Collaborate with physicians and other healthcare professionals to ensure seamless care coordination.
  • Monitor patient progress, making adjustments to care plans as needed.
  • Participate in clinical administrator on-call rotation.

Requirements:

  • ANCC certification
  • Current NY State RN License and Advanced Practice Nurse Prescriber License
  • Master's degree in nursing with completion of accredited program for Nurse Practitioners
