About the Role:

Matrix Medical Network is seeking a skilled Nurse Practitioner to join our team on a PRN/part-time basis. As a Nurse Practitioner with our organization, you will have the opportunity to make a meaningful impact on the lives of our patients by conducting comprehensive health assessments in the comfort of their own homes.

Responsibilities:

Conduct thorough health assessments of patients in their homes, reviewing their medical history, medication adherence, social environment, and other risk factors.
Collaborate with physicians, case managers, and other healthcare professionals to develop personalized care plans for patients.
Provide education and support to patients and their families to promote healthy behaviors and improve health outcomes.
Work collaboratively with our team to ensure seamless care coordination and communication.

Requirements:

Master's degree in Nursing or related field required.
Current RN and NP licensure in the state of practice, with prescription authority or ability to obtain.
Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty.
Minimum of 1 year experience as a Nurse Practitioner.
Strong computer skills and familiarity with electronic health records software.
Excellent verbal and written communication skills.
