Job Title: Nurse Practitioner

We're seeking a highly skilled and compassionate Nurse Practitioner to join our team at ChenMed. As a key member of our clinical operations team, you will provide direct patient care in our medical centers, nursing homes, skilled nursing facilities, and home settings.

Responsibilities:
  • Assess and manage acute and non-acute clinical problems in geriatric patients
  • Perform physical assessments, patient histories, and develop patient management plans
  • Order and interpret laboratory and diagnostic studies, and prescribe medication as needed
  • Collaborate with multidisciplinary teams to ensure patient care needs are met
  • Participate in patient care rounds and conferences, and communicate patient management strategies to team members
Requirements:
  • Current RN licensure and Nurse Practitioner Certification in the State of practice
  • Board certification by AANP or ANCC is preferred but may be required for certain States
  • Current DEA number from the DEA for schedule II-V controlled substances may be required based on State of practice
  • A minimum of 1 year of clinical experience preferred, but willing to consider strong new graduates
