Job Title: Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Hampton Roads Community Health Center, Inc.

Job Summary:

The successful candidate will provide high-quality primary care services to patients of all ages, including diagnosis and treatment of acute and chronic health problems, as well as preventive care focused on health risk factor reduction.

Key Responsibilities:
  • Provide primary care services, including diagnosis and treatment of common acute and chronic illnesses, minor trauma, and episodic problems.
  • Perform minor surgical procedures and initiate life-saving procedures in emergencies.
  • Plan diagnostic studies, evaluate results, and consult with supervising physicians as necessary.
  • Write prescriptions for required medication and/or treatment, as allowed by state regulation and/or clinical protocols.
  • Refer complex and high-priority cases to the physician on duty.
  • Complete Electronic Medical Records according to practice standards and close charts within the appropriate time frame.
  • Serve as a preceptor for medical and nursing professionals and other students.
  • Utilize medication assistance services as appropriate.
Requirements:
  • Licensed to practice as a Nurse Practitioner in the Commonwealth of Virginia, with prescriptive authority.
  • Three to five years of related experience preferred.
  • Basic Life Support Certification required.
  • Family Nurse Practitioner Certification desirable.
