Key Responsibilities
  • Provide Cross-Continuum Care Coordination: You will work closely with our healthcare team to ensure seamless transitions of care for our patients, from admission to discharge and beyond.
  • Conduct Psychosocial Assessments: You will assess the social and emotional needs of our patients and their families, identifying opportunities to provide support and connect them with community resources.
  • Develop and Implement Care Plans: You will work with our patients and their families to develop personalized care plans that address their unique needs and goals.
  • Provide Education and Support: You will educate patients and their families about their conditions, treatment options, and self-care strategies, providing emotional support and guidance throughout their healthcare journey.
  • Collaborate with Healthcare Team: You will work closely with our healthcare team, including physicians, nurses, and other healthcare professionals, to ensure comprehensive and coordinated care.
Requirements
  • Master's Degree in Social Work: You must have a Master's degree in Social Work from an accredited program.
  • Licensed Social Worker: You must be licensed as a Social Worker in the state of Washington.
  • Experience in Hospital Setting: You must have at least three years of experience working in a hospital setting, preferably in a social work or case management role.
  • Strong Communication Skills: You must have excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Knowledge of Healthcare Systems: You must have a strong understanding of healthcare systems, including medical terminology, insurance, and billing processes.
