Job Title: Resident Care Coordinator-LPN in Skilled Nursing

We are seeking a highly skilled and compassionate Resident Care Coordinator-LPN to join our Skilled Nursing team at Senior Living Communities. As a key member of our healthcare team, you will be responsible for implementing standards of resident care on a 24-hour basis, overseeing the nursing aspects, and directing and evaluating the care provided by our skilled nursing staff.

Key Responsibilities:
  • Maintain accurate and up-to-date resident charts, including admission documents, care plans, and records of care.
  • Ensure compliance with all regulations from federal, state, and other governmental agencies, and serve as a liaison between our campus and regulatory agencies.
  • Administer resident medications, assess responses, and make adjustments or recommendations as needed.
  • Participate in and lead staff meetings, implement nursing programs, and conduct daily resident rounds.
  • Participate in ongoing education and training programs, and contribute to a positive and supportive work environment.
Requirements:
  • Current state licensure as a Registered Nurse or Licensed Practical Nurse.
  • Certification in CPR, AED, and First Aid.
  • Administrative and management experience, with a minimum of two years in a similar healthcare position.
  • Ability to read, write, speak, and understand English fluently, and meet or exceed the company's attendance and punctuality standards.
