Job Summary

The Home Care Licensed Practical Nurse is a vital member of our healthcare team, providing direct nursing care to patients in their homes. As a LPN Visit Nurse, you will work under the supervision of a Registered Nurse and/or designated supervisor to deliver high-quality patient care.

Responsibilities

  • Provide High-Quality Clinical Services
    • Deliver nursing care as prescribed by the physician to acutely ill and/or chronically ill patients in their homes.
    • Record clinical and progress notes, and coordinate other patient care activities/services to provide the highest quality of patient care.
  • Document Patient Visits
    • Document patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record.
  • Participate in Care Coordination
    • Actively participate in the agency's care coordination process, including timely reporting patients' needs and changes in condition.
    • Communicate effectively with the patient, family, physician, other clinicians, agency management staff, and case coordinator.
  • Ensure Confidentiality
    • Ensure confidentiality of patient health information per agency policy.

Requirements

  • License Requirements
    • Must have current LPN licensure in state of practice.
  • Experience Requirements
    • Previous home care experience preferred.
