About the Role
- We are looking for a skilled and compassionate Nurse Practitioner to provide direct patient care in various settings, including medical centers, nursing homes, and home settings.
- You will be responsible for conducting geriatric assessments, developing nursing plans of care, and providing health education to patients and their families.
- The ideal candidate will possess strong critical thinking and problem-solving skills, excellent communication and interpersonal skills, and the ability to work well under pressure.
Responsibilities
- Assess and manage acute and non-acute clinical problems, perform physical assessments, and develop patient management plans.
- Provide patient education and counseling, referring patients to specialists when necessary.
- Document patient progress and maintain accurate records, adhering to ChenMed's standards for quality, service, productivity, and teamwork.
- Promote healthy behaviors and prevent disease through early detection and intervention.
- Collaborate with multidisciplinary teams to ensure seamless patient care and communicate effectively with physicians, nurses, and other healthcare professionals.
Requirements
- A minimum of 1 year of clinical experience as a Nurse Practitioner, preferably in gerontology or primary care.
- Current certification as an Advanced Registered Nurse Practitioner (ARNP) or equivalent.
- Board certification by AANP or ANCC is preferred but may be required depending on the state of practice.
- Basic Life Support (BLS) certification from the American Heart Association or American Red Cross within the first 90 days of employment.
