We are seeking a skilled Nurse Practitioner to join our team at HealthCare Support. As a Nurse Practitioner, you will be responsible for providing high-quality patient care in a variety of settings.
Daily Responsibilities:- Conduct comprehensive assessments and develop effective care plans.
- Manage acute and chronic medical and behavioral conditions.
- Ensure accurate and complete documentation, including ICD 10 conditions.
- Communicate and collaborate with the interdisciplinary care team.
- Provide patients and caregivers with counseling and education.
Monday - Friday, 8:00 AM to 5:00 PM.
Required Qualifications:- Active and unrestricted license in the state of Missouri.
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
- Minimum 1 year NP experience.
- Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following: Family Nurse Practitioner, Adult Nurse Practitioner, Gerontology Nurse Practitioner, or Adult-Gerontology Acute Care Nurse Practitioner.
- 1+ year of clinical experience in practice (long-term care setting preferred).
- Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs.
- Understanding of Geriatrics and Chronic Illness.
- Understanding of Advanced Illness and end of life discussions.
- Proficient computer skills including the ability to document medical information with written and electronic medical records.

$70 - $80 an hour.
