Position Summary:

The Justice Involved Community Navigator is expected to possess foundational knowledge of mental health and addiction recovery processes, as well as familiarity with the criminal justice system. This role necessitates experience with community resources and referral systems, along with the ability to collaborate effectively with community-based providers and agencies while delivering exceptional service. The Navigator will manage a caseload of approximately 10-15 clients and utilize CRM software to track various required tasks.

Key Responsibilities:
  • Conduct interviews with clients, perform basic assessments, and engage them in treatment and ongoing care planning, including in correctional facilities.
  • Develop and implement case management service plans.
  • Facilitate psychoeducation groups within correctional settings as needed.
  • Collaborate with clients and their support systems to prepare individualized care plans.
  • Establish connections with community resources to assist clients in building effective recovery support networks.
  • Ensure compliance with regulatory requirements and industry standards through the development and management of appropriate systems.
  • When necessary, accompany clients to community resources and services as outlined in their care plans.
  • Assist clients in developing transportation and other resources to promote self-management of their care.
  • Engage in staff supervision and participate in training programs.
  • Attend team meetings to discuss client progress.
  • Maintain regular communication with referral sources, advocacy groups, and community resources.
  • Ensure patient records are maintained in accordance with Gosnold standards.
  • Work effectively within a multidisciplinary team environment.
  • Fulfill outreach responsibilities using personal transportation.
  • Perform additional duties as assigned.
Qualifications:

Education: A Bachelor’s degree in counseling, psychology, social work, nursing, or a related field is preferred. Two years of experience in an addiction program may substitute for academic qualifications.

Experience: A minimum of two years of professional or lived experience in mental health and addiction is required.

Skills: Strong interpersonal and organizational skills, self-motivation, and proficiency with electronic health records are essential. Comfort in engaging with individuals involved in the justice system and working in various correctional facilities is necessary.

Certificates, Licenses, and Registrations: A valid, clean Driver's License is required.
